Join us for NYSPHA's interactive pre-conference session to learn what roles hospitals, health care, health departments and the business community are doing to improve the health of the population.  Many traditional and non-traditional relationships are being built around the country, and investments are being made to address the social determinants of health through a variety of unexpected sources.  How can your agency get involved?  How do we find funding?  How do we capitalize on a rapidly-changing healthcare environment?

By attending this session participants will:

  • Learn how to best secure resources for improving population health, including chronic disease prevention
  • Learn about the latest funding opportunities and non-traditional public health partners
  • Gain a better understanding about opportunities for promoting population health in a reforming healthcare system
  • Hear stories highlighting the work hospitals around the U.S. are doing to promote population health
